On this site, you may come across Adobe® Portable Document Format (PDF) documents or Microsoft Word (DOC) documents.

We make all efforts to offer the form as a PDF so that all users can use the documents, but there are some cases where we received enough feedback that using a Word document was necessary in order to offer flexibility. To view a PDF, you will need to download Adobe's free Acrobat Reader.

Download the latest version of Adobe's free Acrobat Reader

Fill out a document:

  1. Click on the link to open a form.
  2. Place your cursor where you want to add information (or print the document to fill out by hand).
  3. Type in as much information as you need.
  4. When you are done, you can print and/or save the document to your computer hard drive.

Save the document:
To save your changes in a PDF document, you must have Acrobat Reader version 8.0 or higher. Otherwise, you will be able to save the PDF but all changes entered will be lost.

  1. Once you are finished filling out a document, click on "File" from the top left of your computer screen.
  2. Click "Save As".
  3. Choose the location on your computer where you want to save the form (example: Desktop).
  4. Click "Save".
