Help Downloading Documents

On this site, you may come across Adobe® Portable Document Format (PDF) documents or Microsoft Word (DOC) documents.
We make all efforts to offer the form as a PDF so that all users can use the documents, but there are some cases where we received enough feedback that using a Word document was necessary in order to offer flexibility. To view a PDF, you will need to download Adobe's free Acrobat Reader.
Download the latest version of Adobe's free Acrobat Reader
Fill out a document:
- Click on the link to open a form.
- Place your cursor where you want to add information (or print the document to fill out by hand).
- Type in as much information as you need.
- When you are done, you can print and/or save the document to your computer hard drive.
Save the document:
To save your changes in a PDF document, you must have Acrobat Reader version 8.0 or higher. Otherwise, you will be able to save the PDF but all changes entered will be lost.
- Once you are finished filling out a document, click on "File" from the top left of your computer screen.
- Click "Save As".
- Choose the location on your computer where you want to save the form (example: Desktop).
- Click "Save".
